Pursuant to Art. 13 of the General Regulation on Personal Data Protection of April 27, 2016 (Journal of Laws EU L 119 of 04.05.2016) we hereby inform that:
1. the administrator of your personal data is Formika Dzieciom Foundation.
2. to contact the Data Protection Office, call: 690 000 261 or send an mail to: fundacja@formikadzieciom.org.pl
3. your personal data will be processed to carry out statutory objectives of the foundation and provide you with greetings – under Art. 6, item 1 letter e of the General Regulation on Personal Data Protection of April 27, 2016;
4. the category of personal data: non-sensitive data;
5. your personal data have been obtained from the application form, referred to hereinabove;
6. the recipients of your personal data will be solely the entities authorized to be provided with the personal data, under relevant laws; 7. your personal data will be stored until the objection has been raised;
8. you are entitled to: claim the administrator to give you access to personal data, have them amended, removed or limit the processing, raise an objection against the processing in purposes other than the statutory ones;
9. you have right to lodge a complaint to the supervisory body.
